I, the undersigned, being the parent or legal guardian of the above-named child, acknowledge that I am voluntarily allowing my child to participate in the Children’s Pamper Party, including but not limited to activities such as mini-manicures, facials, and other pampering activities.
I understand that despite all reasonable precautions being taken to ensure the safety of my child, there are inherent risks involved in any event or activity. By signing this waiver, I assume all risks associated with my child’s participation in the event and agree to release and hold harmless Le Petit Glam Bus, its employees, agents, and volunteers from any liability for any injury, loss, or damage that may occur as a result of my child’s participation.
I acknowledge that: I have been informed of the nature of the activities involved and have had the opportunity to ask questions.
My child is in good health and capable of participating in the activities planned for the event.
I consent to any medical treatment that may be necessary in the event of an emergency.
Photo and Video Release: I grant permission for Le Petit Glam Bus to use photographs and/or video recordings of my child taken during the event for promotional purposes, including but not limited to social media, websites, and print media. I understand that no compensation will be provided for these images.
I understand that Le Petit Glam Bus uses various beauty products and procedures that may contain allergens or irritants. I agree to disclose any known allergies or sensitivities to the them. I understand that it is my responsibility to inform the staff of any changes to my allergy information before the party. I acknowledge that Le Petit Glam bus will take reasonable precautions, but I release them and it's staff from liability in case of any allergic reactions or adverse effects resulting from beauty services.
By signing below, I acknowledge that I have read and understood this waiver and release form, and I agree to Le Petit Glam Bus terms and conditions.
We love capturing special glam moments! Please let us know if you would prefer that photos/videos of your child not be shared on our website or social media.
